Getting a successful surface piercing

Surface piercings are notorious for their tendency to migrate or be rejected because they are close to the skin’s surface and because jewelry, including most types of barbells, tends to exert a fair amount of force on the surface piercing no matter how well done the piercing is. So what do you do to try and achieve an attractive, successful surface piercing?
